Heart is a vital organ of our body that pumps blood through the blood vessels of the circulatory system. It not only provides oxygen and nutrients to the body but also helps in the removal of metabolic wastes. It is very important to keep the heart healthy for a healthy life.
But unhealthy eating and lifestyle can lead to various cardiac ailments. One of the most common heart disorders is CAD (Coronary artery disease, also called coronary heart disease) that occurs due to plaque build-up in the arteries.
What is CAD?
CAD is a disorder in which the coronary arteries become narrow due to accumulation of fatty material within the inner walls of the arteries. The narrowed arteries become rough and result in limited supply of the oxygen rich blood in the heart muscle leading to heart attack.
The two diseases of coronary artery are:
Arteriosclerosis involves thickening or calcification of coronary artery that leads to its stenosis.
Atherosclerosis is the formation of yellowish plaques of cholesterol inside the lumen of the artery that blocks the artery.
Symptoms of Coronary Artery Disease
Coronary artery disease is a silent killer that does not show any symptoms initially when the disease is in its initial stage. But the damage continues to occur until the arteries get blocked.
In later stages, following symptoms of coronary artery disease (CAD) may occur:
- Chest pain or angina that may be described as any of the following:
- Shortness of breath
How Is Coronary Artery Disease Diagnosed?
CAD is suspected in almost all the cases of chest pain. Its confirmation requires thorough physical examination and medical history of the patient. In addition to this following diagnostic tests are also performed to know the extent of the coronary heart disease, its effect on the heart, and accordingly the best treatment is advised:
- An electrocardiogram (ECG or EKG)
- Stress tests
- Electron beam (ultrafast) CT scans
- Cardiac catheterization
Heart Bypass or CABG for CAD
In cases of early stage CAD, lifestyle changes and some medications may help the patient to keep the disease in control. But if the damage has already occurred then CABG or Coronary artery bypass commonly known as Heart Bypass or Cardiac bypass graft surgery is required to treat CAD.
The procedure involves harvesting and grafting of the vessel from some other part of the body over the blocked coronary artery. Thus a bypass of blood flow is created through the blocked coronary artery to restore the flow of blood over the heart muscle.
Depending upon the number of bypassed arteries, CABG can be classified as single, dual, triple or quad bypass surgery. The most commonly grafted vessels are:
- Internal mammary artery (chest)
- Saphenous vein (leg)
- Radial artery (wrist)
- Gastro epiloic artery and inferior Epigastric artery (abdomen)
CABG can be performed by any of the following techniques:
Traditional heart bypass requires a long incision
Traditionally, CABG is performed as an open heart surgery done under general anaesthesia through a long 6-8 inch incision on the chest. During the procedure, the heart of the patient is stopped and the patient is put on the heart-lung bypass machine. Meanwhile, the surgeon harvests an apt vessel and the graft is implanted over the occluded coronary artery.
Off-pump CABG is similar to open procedure but the patient is not put on the bypass machine. The heart beat is slowed down using certain drugs and graft is implanted on functioning heart.
The most advanced minimally invasive or robotic CABG requires only a 3 inch incision through which the internal anatomy of the chest is visualized on the monitor outside and the graft is implanted.
What to expect from CABG Surgery?
- Reduced chest pain
- Restores the effective blood supply to the heart
- Improved capability of carrying out the exercises and other strenuous works
- Increases the life span
However, it is important to note that CABG or Heart bypass is the not the final solution to treat CAD. One should make lifestyle changes to prevent further blockages in the vessels.
